Job Title: Program Monitor

Duration: 6 Months+
Job Descriptions:
Role Description & Job Duties
  • Monitors and provides consultative services to Supports Coordinators and providers for the Adult Autism Waiver (AAW) to assure compliance with state regulations, policies, and procedures
  • Evaluates the quality of AAW services and supports through on-site visits and review of records
  • Interviews and meets with AAW consumers to ensure needs are being addressed
  • Conducts annual quality assurance monitoring of AAW providers and participates in the development, coordination, and implementation of corrective action plans
  • Reviews, approves, and monitors Individual Support Plans (ISP)
  • Provides general case management services for AAW participants
  • Makes recommendations on how to improve programs and service delivery
  • Determines final non-financial eligibility for the AAW
  • Meets with service providers to explain waiver services, policies, and regulations and work with providers to ensure compliance with the AAW
Minimum Qualifications
  • • A bachelor's degree is preferred, but not required.
  • A combination of education, experience, and training, which may include experience working in a public or private human service program with at least 12 college level credits of education in a social science or human services, education, administrative experience such as case management, service delivery, data collection, or program monitoring/quality assurance experience will be considered in lieu of a bachelor's degree
  • Intermediate Computer skills to include, but not limited, to Microsoft Office Products.
